Requires a Bachelor's degree in social work or a related field and 4 years of case management experience. Must possess a valid driver's license and the ability to obtain child abuse and neglect mandated reporter certification.

Across our 60+ locations, we provide programs in a variety of service areas, including workforce development, children, youth, families, seniors and homeless prevention.
· Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries and follow-up.
· Manage a case load and monitor and document client progress toward service plan goals.
· Monitor risk and assess the status of current service plans as well as update/ revise the service plans as the client's needs change
Deep cleaning, home health services, legal services, mental/ medical care
· Utilize motivational interviewing in order to complete assessments and service plans
· Ensure that services plans are completed within 30 days and service plans reviews are completed every 6 month
· Maintain familiarity with program resources available on- and off-site.
· Travel all 5 boroughs in NYC.

· Coordinate client services with other social service, mental health, medical, employment, educational, child care and other providers.
· Conduct home visits as required by contract and weekly telephonic communication.
· Complete program reports on a monthly basis, or more frequently as required.
· Monitor risk and assess the status of current service plans as well as update/ revise the service plans as the client's needs change.
· Seek appropriate guidance from the supervisor in situations requiring clarification and consultation.
· Review, and sign off on, service plans and psycho-social assessments as assigned.
· Report to, and meet with, supervisor on a monthly basis.
· Complete initial home visits within 24-48 hours of assignment
· Document all communication with clients within 24 hours in ETO database
· Become a BronxWorks Approved Driver in order to drive agency vehicle. Must be comfortable with operating agency vehicle
· Perform additional duties as assigned.
· Bachelor's degree in social work or other related fields of study.
· 4-years of experience in case management or in another related field of work.
· Certified child abuse, and neglect mandated reporter, or ability to obtain certification within 3 months of employment.
· Enrollment in MSW program preferred.
· Possess a clean valid driver’s license and be willing to drive a company vehicle.
· Engaging personality and detailed orientated skills are essential.
· Strong oral and written communication, time management and organizational skills are necessary.
· Proficiency in Microsoft Office suite and other standard business technology is required.

· Ability to use a computer for prolonged periods.
· Ability to occasionally lift and/or move up to 10 pounds.
· Ability to stand, walk, or sit for long periods of time.
· Ability to climb five flights of stairs, if required, to conduct home visits and/or fieldwork.
· Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
· Ability to bend and retrieve objects and/or documents.
