JOB REQUIREMENTS Board

• Experience: 2 years preferred SUBMISSION REQUIREMENTS All required documentation must be submitted no later than 5 business days prior to start Provider questionnaire required at time of submission NPDB self-query required at time of presentation, within 6 months Full disclosure on all malpractice required Do not send open-ended presentations If the role includes specific coverage dates, your submission must clearly state exact dates the provider can work No follow-up required—if we have to ask for availability, the submission will be rejected Submissions without rates will be rejected immediately

Provide sporadic solo night coverage Perform regional block (nerve block) anesthesia for high volume of ortho patients Provide OB anesthesia coverage Deliver high quality, patient centered care across a broad range of surgical and procedural areas Cover a 1:3 CRNA/room ratio
