Provide skilled nursing care to patients and their families within a home health setting to promote recovery and prevent hospitalization. Deliver services under the supervision of a Registered Nurse according to physician orders and individualized care plans.
Must be a qualified Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN). Ability to provide compassionate, high-quality care as part of an interdisciplinary team is required.
We are seeking a compassionate and qualified Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in a Home Health setting. This role supports patients and their families in the home by delivering high-quality, patient-centered care aimed at promoting recovery, maintaining health, and preventing hospitalization. The LPN/LVN provides nursing services under the supervision of a Registered Nurse and in accordance with physician orders and the individualized plan of care developed by the interdisciplinary care team.
