Provide comprehensive mental health assessments, diagnoses, and treatment planning for individuals with severe mental illness and substance use disorders. Collaborate with a multidisciplinary team to coordinate care, conduct outreach, and provide crisis intervention within a community-centered model.
Must be a licensed clinician (LPC, LCSW, or LMFT) with a current CPR certification and the ability to pass background checks. Experience with severe and persistent mental illness (SPMI) and CADC certification are highly preferred.
