Must be a licensed Physical Therapist in Massachusetts with a degree from an accredited program and current CPR certification. Home health experience and a valid driver's license with reliable transportation are required.
We’re looking for a compassionate and skilled Physical Therapist to join our growing home health team. What You’ll Do Provide in-home physical therapy services, including evaluations, start of care visits, follow-ups, recertifications, OASIS assessments, and discharges Develop and implement personalized treatment plans based on patient needs and physician orders Guide patients through therapeutic exercises, gait training, balance and coordination activities, and mobility improvement Educate patients and caregivers on safety, fall prevention, and daily living strategies Monitor patient progress and adjust treatment plans to achieve optimal outcomes Assess vital signs as needed and communicate any concerns to the care team Recommend and educate on the use of assistive devices and durable medical equipment (DME) Collaborate with an interdisciplinary team including nurses, occupational therapists, speech therapists, and physicians Complete timely and accurate documentation Support discharge planning and ensure continuity of care What We’re Looking For Licensed Physical Therapist in the state of Massachusetts Degree from an accredited Physical Therapy program CPR certification (current) Valid driver’s license and reliable transportation Home health experience required Strong communication, organization, and clinical decision-making skills Preferred At least 1 year of clinical experience Why Join Us?
