• Community Care is hiring a Case Manager/Care Manager Day Shift for our Family Care Program in Milwaukee, WI.
• Offering an $8,000 sign-on bonus!
• This is a Monday through Friday position, 8:00 a.m. to 4:30 p.m.

• Case Manager/Care Manager Job Responsibilities: · Manage caseload of Family Care Program members · Complete comprehensive clinical assessments · Working collaboratively to develop, member centered care plans, provide on-going care coordination, member education, quality assurance and monitoring of services. · Travel, Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
• Case Manager/Care Manager Job Requirements: · Education: Bachelor's Degree in Human Services or closely related field required. · Licensure: Current long-term care functional screener certification preferred (eligibility for this certification is required) · Experience: Minimum of 3 years recent experience with older adults or persons with physical or intellectual disabilities is required.
• 3 years of work in community nursing preferred.
