The Adult Behavioral Health Home Coordinator / Community Integration Specialist supports adults in building the skills and accessing the resources needed for successful, independent living. This role emphasizes holistic, person-centered care guided by SAMHSA’s 8 Dimensions of Wellness—emotional, social, physical, financial, and more.
You’ll work as part of a multi-disciplinary team, using psychiatric rehabilitation and integrated care models to support both mental and physical health outcomes.
What You’ll Do Build strong, therapeutic relationships with clients—including those who may be harder to engage Help clients set and achieve personalized wellness goals Provide skill-building and psychoeducational support (individual & group settings) Coordinate care plans and connect clients to essential community resources Support clients and families in making informed decisions Assist with access to medical, behavioral health, and social services Provide crisis intervention when needed Maintain accurate documentation and meet service delivery standards What We’re Looking For Strong interpersonal and communication skills Passion for mental health, community support, and advocacy Ability to work collaboratively in a team environment Flexibility and willingness to meet clients where they are (including community-based work) Experience with behavioral health, case management, or human services preferred Work Environment This is a dynamic, community-based role requiring travel to client homes, service sites, and community locations. Work may be conducted in-person or via telehealth, depending on client needs.
