We are seeking a detail‑oriented Patient Care Coordinator for a Full‑Time position working in a clinical setting. As a member of the healthcare team, the Patient Care Coordinator provides comprehensive patient care management support and plays a key role in the team‑based care model.
The Patient Care Coordinator will
Pre‑flow the daily appointment schedule to identify immunizations, annual physicals, well‑child visits, and due or past‑due medical issues.
Review patient visit summaries and charts to identify due or past‑due healthcare items.
Collaborate with care team members to identify care needs and maintain tracking databases for CBI, QIM, HWFL, IHA, SHA, WCV, DRS, and overdue health maintenance.

Retrieve and track medical forms (ACT, SHA) and forward them to Medical Records.
Update Healthy Weight for Life, Medication Reconciliation, and Asthma Action Plan information into the Care Team Huddle Report.
Prepare required reports, exam results, physician notes, and related documents prior to patient visits.
Facilitate and log labs, X‑rays, consults, referrals, and related reports.
Conduct quarterly audits for ABN and Medicare Medical Necessity.
Identify and add health coaching opportunities to the Care Team Huddle Report.
Manage patient recall lists and schedule patients accordingly; review needs for IZ, CHDP, IHA, DRS, and specialty clinics.
Develop collaborative working relationships with departments to ensure cohesive communication and direction.
Provide patient education and pre‑ and post‑care instructions based on provider orders and care plans.
Perform other related duties as assigned.
What you need
High school diploma or equivalent
Minimum of 3 years’ experience in a clinical setting
Medical Assistant preferred
Knowledge and experience with medical and dental terminology
Current BLS/ALS certificate
Must pass Occupational Health Screening
Must possess a valid form of identification per I‑9 requirements (E‑Verified)
Clean DMV driving record and MACT auto insurance driving eligibility
Proficient in problem resolution, detailed analysis, and accuracy
Ability to work independently and submit weekly progress reports
Ability to prioritize tasks and complete assignments in a timely manner
Ability to work as part of a professional team
The position requires an awareness and keen appreciation of American Indian traditions, customs, and socioeconomic needs and the ability at all times to meet and deal effectively in contacts with Indian health organizations which requires tact, courtesy, discretion, resourcefulness and good judgment in handling functions of a sensitive nature.
