Provide physical therapy services to adult and geriatric patients while establishing customized plans of care. Responsible for maintaining documentation, progress notes, and discharge summaries across various centers.
Must possess a valid State Physical Therapist License and credentials. Experience in Skilled Nursing Facilities (SNF) or Acute care settings is required.
We are hiring a PRN Physical Therapist to deliver care to our patients and work with a diverse team of clinical professionals.
Provide physical therapy to adult and geriatric patients Prepare documentation, progress notes, and discharge summaries Establish a plan of care in accordance with patients' needs and capabilities Travel to nearby centers as needed
