The MDS Coordinator conducts comprehensive assessments for participants prior to enrollment and annually per state guidelines. They are responsible for inputting data into the State OPTS system and managing Participant Plans of Care in the electronic medical record.
Candidates must have at least 3 years of clinical experience as an RN or LPN. Required qualifications include a diploma or associate's degree from an accredited LPN program, a current Louisiana State license, and BLS certification.
The MDS Coordinator will conduct a comprehensive assessment (Minimum Data Set Questionnaire) of all Participants prior to enrollment and on an annual basis according to State Guidelines, will input information into the MDS into the State OPTS system and develop or update the Participant Plan of Care in the electronic medical record.
