The provider will deliver primary care services, including physical exams, diagnosis, and treatment for chronic and acute conditions, to residents in skilled nursing and long-term care facilities. Responsibilities also include ordering tests, prescribing medications, and collaborating with interdisciplinary teams while traveling to various sites.
Candidates must possess an active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license, along with a state-specific license in good standing for Texas. A minimum of 6 months of experience in long-term care or a skilled nursing facility is required, alongside reliable transportation for extensive travel.
Provide primary care services to residents in skilled nursing and long-term care facilities Perform physical exams and medical history assessments Diagnose and treat chronic and acute health conditions Order and interpret diagnostic tests and lab results Develop and adjust individualized care plans Prescribe medications and monitor treatment effectiveness Collaborate with facility staff and interdisciplinary teams Educate patients and families about treatment options and care goals Ensure a safe, respectful, and supportive environment for residents Travel regularly to multiple nursing homes within the designated region to deliver on-site care Maintain a flexible schedule to accommodate facility visits and optimize patient coverage
