This position offers flexibility and the opportunity to provide high-quality care to both short-term rehabilitation and long-term care residents. Position Responsibilities: Provide evidence-based therapeutic interventions under the direction of a Physical Therapist Deliver care to post-acute and long-term care patients in a Skilled Nursing Facility setting Assist in developing and implementing individualized treatment plans Accurately document patient progress and treatment outcomes Collaborate effectively with a multidisciplinary team, including therapists and restorative nursing staff Schedule: PRN / As-Needed Position Flexible scheduling
