The role involves providing case management for adults with severe and persistent mental health issues, coordinating and monitoring treatment plans, and offering individual/group support using the recovery model within the ACT program. Additionally, the Case Manager will provide on-call crisis intervention services covering evenings, nights, weekends, and holidays, and consult with community agencies and families.
Candidates must possess a Bachelor's degree in human services, Psychology, Social Work, Sociology, or a related field, and be proficient with computers, including preferred experience with Electronic Health Records. A valid driver's license is required for travel throughout the service county, along with strong communication skills and attention to detail.
