The role involves providing high-quality primary care to pediatric and adult patients, developing care plans in collaboration with the team, and ensuring cost-effective delivery of services. Responsibilities also include managing referrals, maintaining electronic medical records efficiently, and partnering with the Behavioral Health team.
Candidates must possess the required degree and licensure to practice as a Nurse Practitioner or Physician Assistant in New York, along with a minimum of two years of relevant experience, preferably in urgent care settings. Outstanding clinical judgment, patient-centric communication skills, and a passion for serving underserved populations are essential.
English and Spanish. Minimum of 2 years relevant experience; experience working in emergency, urgent care or walk-in clinic desirable. Outstanding “bedside manners”: patient-centric with empathy, excellent active listening and communication skills, engaging and efficient. Outstanding clinical capabilities and judgment. Focused on collaborative team-based care. Responsive and capable of working in a high accountability environment. Comfort level with using an electronic health record. Flexible. Excellent judgment and ability to prioritize resources where they will be most useful. Comfortable receiving and providing constructive feedback. Engaged in learning and continuous quality improvement. Outstanding written and verbal communications skills with a culturally diverse patient and staff population. Passionate about providing high quality primary health care to low-income, under-served residents and those lacking access to health care.
