Provide integrated behavioral health care to adults, children, and families using evidence-based treatment models. Coordinate care with primary health teams and external agencies while documenting all interventions in electronic medical records.
Requires a Master's Degree in Social Work and a current New York State LCSW license. Candidates must be bilingual in Spanish and English with at least 3 years of clinical experience treating families and children.
Applies high quality, appropriate short term treatment based on clinical diagnosis and assessment of the patient’s presenting problem. Provides short term treatment modalities at Open Door site including, but not limited to, individual, family, and group counseling. Accurately documents all clinical interventions, diagnosis, symptoms, presenting problems in the Electronic Medical Record. Works with patient to develop treatment plan and goals, based on social, emotional, and health needs of the patient/family. Provides crisis intervention and conducts Warm Hand Off assessments as needed. Incorporates health and wellness education and referrals to preventive care. Coordinates and case manages plans within agency and with outside agencies, including interfacing with school, criminal justice, medical, and social service agencies. Coordinates care with patient’s care team, and communicates regularly with care team. Participates in weekly one hour supervision with LCSW supervisor. Participates in multi-discipline team conferences and service training programs.
