The Clinical Pharmacy Specialist evaluates and dispenses medications based on prescriber orders, while identifying, preventing, and resolving medication-related problems to ensure safe and optimal patient outcomes. Key duties include participating in daily patient care rounds, monitoring drug therapy appropriateness, and performing clinical pharmacy functions like pharmacokinetics and IV to PO conversions.
Candidates must possess a Pharm.D. or equivalent from an ACPE-accredited School of Pharmacy and hold a current, active pharmacist license in the relevant state, with an ASHP accredited residency preferred. Required skills include thorough knowledge of pharmacy laws, proficiency with new medications and technology, effective communication, and the ability to function effectively within an interdisciplinary team.

The Internal Medicine Clinical Pharmacy Specialist evaluates and interprets orders, compounds; and dispenses medications according to the written orders/prescriptions issued by an authorized prescriber. The clinical specialist will also identify, prevent, assess, and resolve medication-related problems to ensure a safe and optimal patient outcome. The clinical specialist will promote, through criteria-based protocols, clinically rational drug therapy and sound pharmaceutical care.
These duties are to be performed in a highly confidential manner, in accordance with the mission, values and behaviors of Insight Hospital and Medical Center. Employees are further expected to provide a high quality of care, service, and kindness toward all patients, staff, physicians, volunteers and guests. DESCRIPTION:
• Thorough knowledge of state and federal laws and regulations governing pharmacy services. Proficient in the use and application of new medications and technology. Effective verbal and written communication skills are required. Ability to effectively participate on an interdisciplinary team or committee. Knowledge of pathophysiology, interpretation of diagnostic criteria, pharmacokinetics and pharmacotherapeutics. Problem solving ability and analytical thinking preferred.
• Ability to work independently and handle stress appropriately. Ability to work cooperatively with other members of Mercy Hospital and Medical Center and pharmacy staff. Ability to handle multiple tasks and adapt to changes in workload and work schedule. Ability to set priorities, make critical decisions, and responds to client concerns. Fosters a team environment. Ability to communicate effectively (verbal and written).
