LPN Home Health

The Licensed Practical Nurse will deliver direct patient care according to approved treatment plans, adhering to clinical guidelines, and collaborating with the RN Case Manager for effective care delivery. Responsibilities also include conducting patient visits, making skilled observations, reporting condition changes, and providing necessary patient and caregiver education.

Candidates must be graduates of an accredited practical nursing program and hold a current state license, along with required CPR or BLS certification. A minimum of one year of nursing experience is necessary, with home care or hospital experience preferred, alongside strong clinical and communication skills.
