The Licensed Practical Nurse will perform basic assessments, participate in care planning, and execute interventions in the PACE clinic and participants' homes under supervision. Responsibilities also include administering medications, implementing primary care orders, and educating participants and caregivers.
Candidates must be graduates of an accredited nursing school with a current LPN license and CPR certification. A minimum of one year of experience working with the frail or elderly population is required, along with a valid driver's license and means of transportation.

The Licensed Practical Nurse Under the direction and supervision of the Clinic Manager or Triage Nurse, uses and practices basic assessment, participates in planning, executes interventions in accordance with the plan of care in the PACE clinic and in participant’s homes, and contributes to the evaluation of individualized interventions related to the care plan. Administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone. Provides education to Senior Community Care participants and caregivers in regards to plan of care, nursing treatments or medications. Communicates participant’s condition to primary care givers, physician and interdisciplinary team
