Discover Capital Area Health Network. Job Summary The Licensed Practical Nurse (LPN) provides clinical and administrative support to providers and care teams. This role includes assisting with patient care, documenting treatment plans, performing clinical procedures within scope of practice, and collaborating with staff to ensure high‑quality, comprehensive care for patients. Essential Functions The following duties are considered essential functions. Provide coordinated nursing care under the direction of a licensed provider. Obtain and document patient health history, vital signs, and clinical information. Assist with examinations, treatments, and procedures within LPN scope of practice. Administer medications, immunizations, and injections as ordered. Perform diagnostic tasks such as EKGs and point‑of‑care testing. Support care management by reinforcing treatment plans and patient education. Document all patient interactions and services provided in the electronic health record. Assist with patient triage, follow‑up calls, and care coordination. Maintain clinical supplies, equipment, and infection‑control standards. Collaborate with providers, nurses, and specialists to support continuity of care. Provide patient instruction regarding medications, treatment plans, and preventive care. Participate in quality improvement activities and clinic workflow optimization. Perform administrative tasks such as scheduling, chart preparation, and documentation review. Perform other duties as assigned.
