The Licensed Practical Nurse will be responsible for completing nursing assessments upon client admission, administering medications according to physician orders, and implementing and monitoring detoxification protocols for adults with substance use disorders and co-occurring mental health conditions. Key duties also include conducting required clinical assessments, providing medication education, and accurately documenting all nursing care provided.
Candidates must possess a current, active, and unencumbered LPN license in Pennsylvania, along with required CPR/BLS, AED, and First Aid certifications. A minimum of one year of nursing experience is preferred, with experience in behavioral health, residential treatment, or substance use disorders being strongly advantageous.
Tapestry, part of Pyramid Healthcare, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our residential treatment team in Lake Ariel, PA. This role supports adults receiving treatment for substance use disorders and co-occurring mental health conditions. As an LPN, you will play a key role in medication administration, detox monitoring, and clinical support. Key Responsibilities Complete nursing assessments upon client admission Administer medications per physician orders and program protocols Implement and monitor detoxification protocols Conduct COWS and CIWA assessments per clinical guidelines Provide medication education and support to clients Coordinate care with medical providers during medication management visits Maintain medical equipment and ensure proper functioning Document all nursing care accurately and within required timelines Maintain confidentiality, professional boundaries, and ethical nursing practice Perform additional nursing duties as assigned
Licensure: Current, active, and unencumbered LPN license in the state of Pennsylvania Certifications: CPR/BLS, AED, and First Aid required Experience: Minimum of one year nursing experience preferred Behavioral health, residential treatment, or substance use disorder experience strongly preferred Skills & Competencies: Strong clinical assessment and critical thinking skills Excellent verbal and written communication Ability to respond effectively in crisis situations Knowledge of relevant regulations (DHS, CARF, Joint Commission, etc.) Reliable, professional, and able to work independently and within a team Work Environment & Physical Requirements Primarily indoor clinical setting Ability to stand, walk, sit, and climb stairs for extended periods Ability to lift up to 25 lbs regularly and 50 lbs occasionally Potential exposure to infectious diseases or psychiatric crisis situations Reliable transportation required About Tapestry & Pyramid Healthcare Tapestry is part of Pyramid Healthcare, a national leader in behavioral health and substance use disorder treatment.
