LPN - Rheumatology - Stamford Sign on bonus

Stamford Health is seeking an LPN for our Rheumatology practice in Stamford, CT! The Licensed Practical Nurse (LPN) coordinates patient care following all established standards of practice within the State and Federal guidelines. Provides staff mentorship. Serves as a resource to both staff and management in problem identification and resolution.

$5,000 sign on bonus available!

• Friday 8am-4:30pm 5 days onsite in Stamford

Major accountabilities / Critical responsibilities

• Reviews schedules and organizes patient low; greet patients, documents patients concerns, questions etc…accompanies patient to exam/procedure room and prepares patients for physical exam. Initial patient interview; reviewing patient medical profile, and documents all required charting in EMR/manual charting as appropriate.
• Obtains and accurately documents patient vital signs, weight: reports any abnormalities to physician. Assists with specimen collection and testing as necessary. Documents pertinent information.
• Assists patients as necessary i.e. walking, transferring, dressing etc.
• Provides minor treatment as directed by physician, assists physicians with minor procedures.
• Provides education as indicated after evaluating the learning needs of the patient and or family.
• As required, schedules patient testing and pre certifies if insurances requires.
• Responds to telephone messages (patient, physician offices etc…)
