• Physical Therapist/ Home Care Case Manager South Dayton, Miamisburg, Germantown, Middletown, Dayton,Englewood, Vandalia, Springfield, Urbana, New Carlisle and Yellowsprings General Summary/Responsibilities: The Physical Therapist evaluates and then directly provides or supervises the provision of physical therapy services to patients in their homes or residences.
• When functioning in the capacity of the Home Care Case Manager (HCCM), the Physical Therapist is responsible for the provision and coordination of all components of a patient’s home care services.
• The Physical Therapist provides care as ordered by the physician, follow policies, procedures, and conditions of participation, and regularly reports to their supervisor.
• The incumbent facilitates the development and implementation of the plan of care as outlined by the physician.
• In addition, the incumbent is responsible for all required documentation related to the care they provide and communicates pertinent information regarding the patient to all appropriate health care team members.

• Type of degree: Bachelors - Graduate of an accredited Physical Therapy Program. 
• Area of study or major: Physical Therapy Licensure/Certification/Registration  Current licensure to practice Physical Therapy in Ohio. 

• Prior job title or occupational experience: Clinical experience should have occurred within the last three years unless currently employed in home care.

• Other experience requirements: N/A Knowledge/Skills  Must be capable of repeated standing, stooping, bending and lifting up to 50 lbs.  Must be able to creatively adapt multiple environments and situations for successful outcomes.  Must be self-directed and able to work with little supervision.  Computer skills preferred.  Home care experience preferred.
