The Clinical Case Manager will provide supportive case management services, including relapse prevention and recovery readiness, to clients in the Scatter Site Return Housing Program. Key duties involve conducting mental health assessments, facilitating support groups, providing counseling and crisis intervention, and maintaining confidential client documentation.
The ideal candidate must possess a bachelor's degree in Social Work or a related field and have experience working with populations facing homelessness, substance abuse, and/or mental health issues. Proficiency in MS Office Suite (Excel, Word, PowerPoint) and MS Outlook is also required.

Join The Doe Fund on our mission to break the cycles of homelessness, addiction, and recidivism. Our flagship Ready, Willing & Able (RWA) program provides paid transitional work experience, transitional housing, advanced occupational training, and comprehensive supportive services to individuals experiencing homelessness. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent affordable and supportive housing, with more than 1,900 units in operation or pre-development. The Doe Fund has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs.
The Doe Fund’s Scatter Site Return Housing Program (SSR) focuses on assisting chronically homeless individuals in their transition from shelters to community living. The services include relapse prevention and crisis intervention using the principles of Harm Reduction and Motivational Interviewing, education on self-advocacy, community integration, safety, money management, socialization, employment skills, and referrals to appropriate treatment facilities. Through the Scatter Site supportive housing program, clients are moved into apartments located in the Bronx, Brooklyn, and Manhattan areas
We are currently seeking a Clinical Case Manager. The person in this role will be responsible for providing supportive case management services including relapse prevention and recovery readiness to the Scatter Site Return clients.
