Work one-on-one with patients in their homes to help them gain the strength and independence needed for daily living activities. The role involves making a significant difference in patients' lives by supporting their ability to remain living in their homes.
Candidates must possess a Bachelor of Science in Physical Therapy, a current state Physical Therapist license, a valid driver's license, and current CPR certification with a negative TB screen. A minimum of two years of experience as a Physical Therapist in an acute care or rehabilitation setting is required.
