The physical therapist will provide home-based services including patient evaluations, assessments, and treatment plans. They are responsible for delivering high-quality care while maintaining accurate documentation.
Candidates must hold a valid Florida State Physical Therapy license and have at least 3 years of experience, including 1 year in home health. Additional requirements include Oasis experience, CPR certification, and malpractice insurance.
