The Social Worker will assess hospice patients to identify their psychosocial, grief, financial, environmental, and community needs. They will provide counseling, crisis intervention, and facilitate access to community resources based on patient and family needs.
Candidates must have a Bachelor's or Master's degree in social work from an accredited program and a current state license. One year of experience as a Social Worker in a clinical care setting is required, with hospice experience preferred.

Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals. Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues. Facilitate community resources based on patient/family needs.
