The Licensed Practical Nurse will provide skilled nursing care to patients in their homes and collaborate with the healthcare team to support patient health needs. They are responsible for accurate documentation, patient education, and participating in an on-call rotation for clinical needs.
Candidates must be a graduate of an approved school of practical nursing and hold a current Indiana LPN license. Two years of experience in home health care is preferred, along with a valid driver's license and reliable transportation.

This part-time LPN role offers a flexible schedule with a few shifts per week. The LPN will provide nursing care to patients, support their health needs, communicate with families and the care team, and complete required documentation. The position also includes participation in an on-call rotation for occasional clinical needs.
