The Nurse Practitioner will conduct in-home health assessments for value-based patient populations within a designated geographic area. Responsibilities include performing annual health assessments, post-discharge visits, and managing care for complex patients.
Candidates must hold an active, unencumbered Nurse Practitioner license and national certification (ANCC or AANP). Applicants must be free of any unresolved disciplinary actions or restrictions against their practice.
The Home Visit Nurse Practitioner will conduct in-home health assessments on Value-based Patient Populations. The Home Visit Nurse Practitioner will complete visits on patients within a geographic area of responsibility. The nature of the home visit can vary and could include performing an annual health assessment, a post discharge visit, or visiting more complex members more than once per year.
NP license and national certification (ANCC or AANP, or the ability to obtain national certification and/or NP license in state of assignment) active, unencumbered licensure free any open/unresolved disciplinary actions including probation or restrictions against privilege to practice. WORK SCHEDULE: Monday - Friday, 8:00AM - 5:00PM
