Ozarks Community Hospital (OCH), located in Springfield, MO has an opening for a Community Health Worker (CHW) within our Medical Home department.
Our Mission: Ozarks Community Hospital and clinics are dedicated to providing exceptional healthcare and preventive services to all patients in an atmosphere of compassion, respect, and dignity, with a commitment to care for the underserved and to improve access to care.
The CHW will be an integral part of the client resource team.
CHW will help to bridge patients to resources to reduce social determinants of health that impact clients' health and well-being.
Provides informal coaching, counseling and social support, culturally appropriate health education, and basic screening services.
Ability to perform duties consistent with Standards of Practice, goals and philosophy of Ozarks Community Hospital and the Employee Code of Conduct.

Requirements
High school diploma or equivalent. Must obtain CHW certification through an approved curriculum within 12 months of hire date. Valid Driver's License required. This position may require a Motor Vehicle Driving Record Background Check to be completed during the onboarding new hire process and throughout employment. Must have a valid Class E license upon hire or Class E license must be obtained during orientation period. Current BLS certification upon hire or BLS certification must be obtained during orientation period or at the next available scheduled class.
