The PRN LVN will perform comprehensive patient assessments, develop individualized care plans, and administer medications while maintaining accurate medical records. They are also responsible for collaborating with the healthcare team to ensure continuity of care and responding promptly to medical emergencies.
Candidates must hold a current Texas Licensed Vocational Nurse license and maintain BLS, ACLS, and PALS certifications. A minimum of one year of nursing experience and a high school diploma or GED are required.
Maintains strong positive communication with physicians and works as a team to enhance the success as a department of HCHD Develops strong interdepartmental teamwork to meet the needs of the patients and physicians. Patient Assessment and Care: Conduct comprehensive assessments of patients' health status, including physical, emotional, and psychosocial dimensions. Develop and implement individualized care plans based on assessment findings and evidence-based practices. Medication Administration: Administer medications and treatments as prescribed by physicians, ensuring accuracy, safety, and adherence to established protocols. Monitor patients for adverse reactions and intervene promptly as needed. Patient Education: Provide education to patients and their families regarding disease management, treatment plans, medications, and preventive care measures. Empower patients to participate in their own healthcare decisions and self-care activities.
Collaborate with physicians, nurse practitioners, therapists, and other healthcare team members to coordinate patient care and facilitate interdisciplinary communication. Participate in patient care conferences and case reviews to ensure continuity of care.
