The Community Health Worker provides care management, health education, and resource referrals to support patients with chronic diseases and social needs. They also act as a bridge between patients and the care team while mentoring and training other staff members.
Candidates must have an Associate's degree and at least 2 years of experience in social services or case management, or a Bachelor's degree in a related field. A Community Health Worker training certificate is required to be obtained within one year of hire.
