The Case Manager provides direct support to individuals with mental health and substance use disorders by coordinating care and teaching daily living skills. This role involves conducting home visits, facilitating community resources, and collaborating with treatment teams to ensure effective service delivery.
Candidates must possess either a High School Diploma with 3 years of relevant behavioral health experience or a Bachelor’s degree in a social sciences field. A valid driver's license, reliable transportation, and an understanding of mental illness and the community mental health system are required.
Coordinates assessments, treatment planning or crisis intervention services. Facilitates healthcare, behavioral health and other services and supports as part of care coordination. Provides training and facilitates linkages for the use of community resources. Monitors symptoms and service delivery to ensure client needs are met. Obtains or assist clients in obtaining necessary services for basic necessities and services. Assists clients in accessing self-help support groups and increasing social support networks. Provides appropriate interventions to aid clients in the restoration of daily living skills. Coordinates and assist in crisis management and stabilization. Collaborates and communicates effectively with treatment team for the coordination of care. Qualifications High School Diploma and 3 years of relevant work experience in behavioral health or Bachelor’s degree in a social sciences field or related field. An understanding of mental illness, psychiatric symptoms, and impact on functioning and behavior. An understanding of how to therapeutically engage a person with mental illness. Valid driver's license, car insurance, and insurable driving record required. Willingness to travel throughout the assigned service area. Must have reliable transportation.
