The Complex Care Nurse develops and implements comprehensive care plans for high-risk patient populations, including those with substance use disorders and chronic conditions. Responsibilities include conducting patient visits, administering medications, and providing health education to move patients toward optimal wellness.
Candidates must be a Registered Nurse in Massachusetts with BLS certification and at least two years of experience in a community health setting. An Associate's degree in Nursing is required, while a BSN and experience in medical case management are preferred.

The Complex Care Nurse will aid in the development of and implement systems for population health-based and complex care nursing services at the top of the incumbent’s license to patients in accordance with all applicable Massachusetts State Laws as well as regulations promulgated by the Department of Public Health and Harbor Health Services, Inc.
The Complex Care Nurse will assist in implementing programs to ensure our High-Risk Patient populations (Substance Use Disorder, Infectious Disease, Complex Care, Geriatric and Chronic Conditions) are properly managed including care coordination, consultations, tests, medication management, and patient education to move toward optimal wellness.
