Requires a Master's degree in social work from an accredited program. Alternatively, a bachelor's degree in social work with at least one year of experience in a healthcare setting is acceptable.

This role partners closely with patients, caregivers, physicians, and the interdisciplinary care team to develop and coordinate individualized plans of care.
Key Responsibilities Assess patient and family needs and develop individualized plans of care Provide counseling, behavioral health interventions, education, and community resource support Coordinate care with physicians and interdisciplinary team members Document patient progress, interventions, and outcomes in accordance with agency standards Conduct hospice information visits for home care patients when requested Support patient and caregiver education and discharge planning Mentor and support new MSW staff and students Bring your care where it matters most—home.
