The Case Manager supports young adults in Rensselaer County by helping them achieve independence and stability through individualized goal setting. Key duties include providing crisis intervention, teaching daily living skills, and connecting clients with educational and employment resources.
Candidates must have a high school diploma or GED with at least two years of relevant experience, or a degree in health/human services with one year of experience. A valid New York State driver's license and reliable transportation are required.
