Pyramid Healthcare is seeking a compassionate and motivated Substance Abuse Counselor / Co-Occurring Specialist to join our Assertive Community Treatment Team (ACTT) in Winston-Salem, NC.
This community-based role focuses on providing direct outpatient Substance Use Disorder (SUD) services to individuals living with serious mental illness and co-occurring substance use disorders.

Position Summary
The Substance Abuse Counselor / Co-Occurring Specialist provides counseling, crisis intervention, treatment planning, and community-based support services to individuals receiving ACTT services. This role works collaboratively with a multidisciplinary treatment team to improve client outcomes and reduce barriers to recovery. 🔹 Key Responsibilities
Provide community-based substance use treatment services in homes and community settings
Conduct individual, group, and family therapy sessions
Complete LOCAs, pre-authorizations, and clinical reviews in a timely manner
Develop and maintain treatment plans, NCTOPPS, discharge plans, and aftercare plans
Maintain ongoing communication with families, referral sources, and support systems

Deliver interventions for individuals with severe and persistent mental illness and substance use disorders
Participate in on-call and emergency service rotation, including crisis response and hospital diversion support
Attend treatment team meetings, supervision, and required trainings
Complete documentation accurately and within required timelines
Perform additional duties as assigned ✅ Required Qualifications
CADC or LCAS required
Ability to obtain required certification/licensure within 90 days of hire (if applicable)
Qualified Professional (QP) status per 10A NCAC 27G.0104
Valid North Carolina driver’s license
Ability and willingness to travel locally throughout the Winston-Salem area 🌟 Preferred Experience
ACTT, community mental health, or outpatient behavioral health experience
Experience working with co-occurring disorders and crisis intervention
