• Key responsibilities
• Conduct screenings, assessments, and ongoing case management for post-adoption and post-guardianship families
• Develop and maintain individualized service plans focused on family stability and child well-being
• Provide support, education, advocacy, and crisis intervention to children, caregivers, and families
• Collaborate with schools, community providers, and interdisciplinary team members to coordinate services
• Connect families with counseling, behavioral health, and community resources
• Maintain accurate and timely documentation, reports, and case records in compliance with agency and OCFS requirements
• Participate in team meetings, supervision, trainings, and on-call responsibilities
• Support agency initiatives, outreach efforts, and evolving program needs

G.A. Family Services (GAFS) Organizational Responsibilities

• Maintain professional, respectful, and collaborative relationships with clients, families, and staff
• Demonstrate ethical and professional conduct while upholding confidentiality standards
• Communicate effectively both verbally and in writing
• Participate in regular supervision and performance evaluations
• Support agency mission, values, and organizational goals
• Uphold child and family rights in accordance with OCFS mandates and agency procedures
• Follow all agency safety policies and procedures
