The coordinator assists patients through post-acute care navigation aiming to positively impact patient outcomes and referral source satisfaction. This involves integrating evidence-based clinical guidelines to develop patient-centered transition plans promoting quality and efficiency in care delivery.
Candidates must be graduates of an approved school of nursing, therapy, or social work and be licensed in the state of practice, requiring a minimum of two years of demonstrated field experience. Essential requirements include understanding Medicare certified home health and hospice regulations and possessing basic technology skills, along with maintaining a valid driver's license and dependable transportation.
As a LPN, you will visit patients in their homes and provide nursing care for them under direction of RNs and in compliance with the physicians' orders.
1. Must possess a valid state driver’s license. 2. Must possess automobile liability insurance. 3. Must wear seat belts at all times while driving. 4. Dependable transportation kept in good working condition. 5. Must be able to drive an automobile in all types of weather conditions. 6. Must be currently licensed as a Licensed Practical Nurse in the State of employment. $2,500 Sign On Bonus
