The Case Manager provides quality crisis residential services, including developing discharge plans and facilitating psychoeducational support groups. They collaborate with multidisciplinary teams to help individuals access community resources and ensure safety within the facility.
Requires a Bachelor's degree in Human Services or a related field and two years of human services experience, including one year of mental health direct care. Must possess a valid driver's license, auto insurance, and Act 33/34 and FBI clearances.

Qualifications Required: Preferred: Education: High school diploma or equivalency AND 12 semester credit hours in Human Services or a related field. Bachelor’s Degree in Human Services or related field Experience: Two years' experience in human services with at least one year of mental health direct care experience. Willingness to work with a diverse client population. Knowledge of local agencies related to mental health, employment, housing, and other community resources. Familiarity with the recovery model. Computer literacy. Other Factors or Requirements: Ability to exercise sound judgment. Time management skills. Written and verbal communication skills. Flexibility in scheduling. Ability to work with team members and accept supervision. Take initiative while being cooperative and empathetic. Maintain strict confidentiality.
