The Physical Therapist provides evaluation and therapy services, including direct patient care and family education, based on practitioner referrals. They collaborate with a multidisciplinary team to design individualized plans of care and manage patient discharge processes.
Candidates must possess the necessary education for a Minnesota PT license and be CPR/BLS certified. While new graduates are considered, one year of physical therapy experience and pediatric experience are preferred.
The pay range for this position is $35.66 hourly to $53.49 hourly. Compensation is determined based on years of experience and department equity.
Upon receiving a licensed independent practitioner referral, implements a systematic patient examination and evaluation utilizing comprehensive compliant and safe clinical care demonstrating the philosophy of family centered care Works with medical staff and the multidisciplinary team to design and implement individualized plans of care to meet the needs of the patient and their family. Makes appropriate discharge plans by collaborating with patients, families and the interdisciplinary team. Functions as a team member and develops leadership skills through activities such as mentoring and providing education to students and other professionals. Supports risk management and compliance initiatives through accurate, thorough documentation. This is not an all-inclusive list of this job’s responsibilities. The incumbent may be required to perform other related duties and participate in special projects as assigned.
