Job description
Overview Physical Therapist – Home Health, Hayward, California.
This travel, temporary assignment starts August 1, 2026 and runs for 13 weeks, with guaranteed 30 hours per week.
Weekly pay ranges from $1,776 to $1,790.
Shifts available: Sunday–Thursday or Tuesday–Saturday within a Skilled Nursing Facility setting.
This open role focuses on clinical assessment and therapy planning for residents in a setting that emphasizes restorative care and functional independence.
Conduct PT evaluations and create individualized treatment plans for residents in a SNF setting.
Provide evidence-based therapeutic interventions to improve mobility, strength, balance, gait, and functional independence.
Document assessments, treatment plans, progress notes, and discharge summaries in compliance with regulatory and facility standards.
Collaborate with nursing, therapy assistants, and other disciplines to coordinate care and support safe transitions between care phases.
Reassess progress regularly and adjust goals and interventions to optimize outcomes; educate patients and families on home programs and safety.
Requirements
Active Physical Therapist license in California or eligibility to obtain licensure.
Degree from an accredited physical therapy program (DPT or equivalent).
Experience in skilled nursing facility and/or home health settings preferred.
Ability to work in a travel assignment and navigate varying schedules; strong documentation and communication skills.
Must be able to work in the specified facility setting and comply with patient privacy and safety requirements.
