The BSW Case Manager facilitates complex discharge planning by collaborating with physicians and healthcare teams to ensure safe patient transitions. They serve as a resource for care coordination and maintain knowledge of community resources and post-acute care options.
A Bachelor's Degree in Social Work from a CSWE-accredited school is required, along with a Virginia RN license or Social Work degree. Preference is given to candidates with two or more years of professional social work or healthcare experience and Case Management certification.

Classification: Non Exempt
4 Year/Bachelor's Degree required - Bachelor's Degree in Social Work from school accredited by the Council on Social Work Education.
2 or more years professional social work experience or two or more years healthcare experience preferred. Case Management or related experience preferred. Refer to the Life Support Training Policy for additional details.
Registered nurse in Virginia or Social Work degree required. Case Management Certification preferred.
Weekend and holiday coverage. Strong problem-solving skills. Ability to apply critical thinking in a variety of situations. Effective verbal and written communication skills with all levels of staff. Basic computer competency. Advanced assessment and teaching skills. Knowledge of medical terminology. Strong Customer Relation skills. Excellent organizational skills with the ability to manage, prioritize, and follow through multiple tasks.
Knowledge/Competence of Microsoft Office products and keyboarding.
