The Peer Health Navigator connects members to mental health and primary care services while helping them develop wellness goals and self-management skills. They serve as a role model of recovery and provide critical engagement and de-escalation during mental health emergencies.
Requires a High School Diploma or GED and a Peer Certification (CPC and AAC in Washington) prior to or shortly after hire. Candidates must have two years of experience supporting individuals with behavioral health challenges and lived experience as a beneficiary of the public Behavioral Health system.
Expected starting wage is $21.05-$25.38. Range ends at $29.70. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
Required: · High School Diploma or GED required
· Peer Certification must be received prior to hire date and must be maintained throughout employment
obtain Certified Peer Counselor (CPC) through HCA prior to hire date and maintained throughout employment and certified by the Department of Health (DOH) as an Agency Affiliated Counselor (AAC) within 60 days of employment.
· Two (2) years’ experience or part-time equivalent supporting individuals with behavioral health challenges
· Experience supporting individuals with physical health challenges to navigate the physical health care system
· Experience as a beneficiary of the public Behavioral Health system of care
· Demonstrates the Telecare mission, purpose, values and beliefs in everyday language and contact with the internal and external stakeholders
· Draws upon own personal recovery experience with mental health/addiction challenges as a basis for engagement as a peer with the members served and to validate members’ own lived experiences
· Assists members served with voicing their strengths, interests, and goals throughout their time in services including: goal plans, crisis plans and other methods of expressing individual preferences
· Utilizes a comprehensive resource guide and educates members about local community supports and resources
· Supports members served in developing strategies to reduce self-stigma
· Engages and communicates with a wide range of members served especially those who are struggling to be active in their own recovery
· Assists participants in all needs including Activities of Daily Living (ADLs)
· Actively participates in multidisciplinary team meetings
· Participates in verbal de-escalation and physical interventions in emergent situations and is willing and able to assist other staff as needed to maintain a safe, healing environment for all
· Travels locally to provide outreach to members served in the community (program specific)
· If employed at Inpatient program, must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training
may be added, deleted and/or changed at the discretion of management.
SKILLS: · Knowledge of recovery focused, strengths based work in mental health/addiction recovery
· Willing to seek to understand members’ unique circumstances, personal preferences, and goals and incorporate them into daily work as well as any needed crisis response to help them regain a sense of control
· Have a strong understanding of recovery and resilience, the value of partnerships with members and member choice, and the balance between protection from harm and personal dignity
· Strong knowledge of relevant community resources and methods for accessing them
· Possess strong communication, writing skills, and typing/computer skills
· Experience working with adults facing mental health/addiction challenges
· Interest or experience in working with people who are difficult to engage and maintain in traditional mental health programs
· Ability to communicate and work effectively with members served, family members, and staff from various backgrounds
