The RN is responsible for admitting patients, developing patient-specific plans of care, and coordinating services with providers and multidisciplinary teams. Key duties include performing assessments, educating caregivers, and overseeing LPNs and Home Health Aides.
Candidates must hold a Nursing Diploma or Associate Degree in Nursing and a valid Connecticut Registered Nurse license. Two years of nursing experience is preferred, along with strong clinical and organizational skills.

The RN is responsible for admission of patients for home care services, development of the patient plan of care, coordination with patient's provider and other disciplines for the provision of services, and safe delivery of patient care.
