The Advanced Practice Nurse will conduct patient visits according to regulations, communicate findings to the interdisciplinary team, and facilitate access to appropriate support models of care. Responsibilities also include consulting on the patient's plan of care, maintaining accurate records, and ensuring collaboration with patients, caregivers, and medical directors.
Candidates must possess a Master’s degree in Nursing (MSN) or a Doctor of Nursing Practice (DNP) from an accredited program. A minimum of one year of experience in Family Practice, General Practice, Geriatric care, or a related field is required, along with an active Nurse Practitioner's license in the state of operation.

Salary Range: $125,000 to $135,000 a year
1. Conduct visits to patients in accordance with local, state and federal regulations. 2. Communicate visit findings to designated members of the interdisciplinary team. 3. Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment). 4. Consult with other members of interdisciplinary team to contribute to the patient's plan of care. 5. Prepare and maintain accurate patient records, charts and documents. 6. Assist in development of clinical practice guidelines/standards in support of quality care as needed. 7. Ensure collaboration with patients, caregivers, medical directors and members of IDT.
