The Case Manager will be essential for client success by coordinating care activities, communicating with external providers, and supporting the admissions process. Key duties also involve facilitating educational groups, developing comprehensive aftercare plans, and maintaining accurate electronic health records.
A Bachelor's degree in a related field is required, with a Master's degree preferred, alongside a minimum of two years of case management experience, ideally in behavioral health or substance use treatment. Candidates must possess strong organizational and communication skills and knowledge of community resources.

We are seeking a dedicated and organized Case Manager to support clients and families throughout the continuum of care. This role is essential to client success and includes coordinating services, communicating with referral sources and external providers, assisting with admissions processes, and supporting aftercare planning to ensure smooth transitions following treatment.
