Reports To: Director of Nursing/Executive Director
We are seeking a Licensed Practical Nurse (LPN) to join our team. The ideal candidate will be responsible for providing high-quality care, ensuring that all standards of care are met, and working collaboratively with other behavioral healthcare professionals. The LPN will provide general clinical care and supervision to residential clients.
Assess and triage client medical needs to determine whether urgent, emergent or stable, and refer clients as appropriate during daily morning sick call and throughout the day as needed.
Refer clients to urgent care, emergency department, dental, PCP, or on-site provider as clinically indicated.
Review medications for all new incoming admissions and update client profile in the medical record.
Complete initial intake including vital signs and weight for all new admissions. Monitor vital signs, weight, blood sugar, pain assessment, etc. as requested by medical and/or psychiatrist as part of client’s treatment plan.
Organize all medication carts in alphabetical order by patient name, discard and/or return any outdated or discontinued RXs.
Document progress note on all client-nurse interactions in the medical record as well as send emails to staff detailing those interactions as necessary.
Consult with admission coordinator, clinical director, and medical provider on admissions with complex medical needs, chronic care problems and/or polypharmacy.
Review process for requesting sick call appointments and medication administration times with new admissions.

Consult with treatment team including counselors, therapists and recovery support specialists on client’s progress and ongoing medical care needs.
Schedule appointments with outside providers and/or specialists as referred by medical provider and/or psychiatric provider. First Aid and medication management
Review and Preparation of Documentation for Disability claims on all clients including any records requests for former clients. Patient Education for diabetes, hypertension, nutrition etc.
