The RN Field Case Manager is responsible for the overall management of a patient's case, coordinating care with clients, families, and other providers. This includes developing patient care plans and documenting activities in the clinical information system.
Candidates must be graduates of an accredited nursing school with a current Texas license and have at least 1 year of hospice or hospital experience. A bachelor's degree in nursing is preferred, along with strong interpersonal skills and knowledge of the hospice industry.

An extended orientation may be considered for an applicant not meeting experience qualifications.
