The Patient Navigator serves as the primary point of contact between patients, referring providers, and clinical caregivers. They are responsible for facilitating care plans, coordinating support services, and empowering patients throughout their cancer treatment.
A high school diploma or GED is required, while an associate degree is preferred. Candidates must have a minimum of 2 years of clinical or non-clinical healthcare-related experience.
SCOPE: The Patient Navigator acts as a link between patients and care providers to facilitate clinical and supportive care services. The role is that of a single point of contact among referring providers, patients, and other clinical caregivers for the purpose of empowering patients in dealing with the disease and the treatment. ESSENTIAL FUNCTIONS: 1. Facilitate Patient and Family Engagement in Care Plans 2. Coordinate Comprehensive Care and Support Services 3. Quality Improvement 4. Outreach and Relationship Management 5.
