Travel assignment providing direct, site-based nursing care for participants in a home health setting
Responsibilities
Provide direct nursing care to participants during home visits, applying advanced nursing knowledge independently
Triage emergent situations and coordinate appropriately with PCPs
Monitor acute and chronic medical conditions in collaboration with Primary Care Providers
Implement and adjust care plans, administer treatments, procedures, and medications as prescribed
Conduct wound care, clinical assessments, lab draws, and other nursing tasks as ordered
Communicate effectively with participants, caregivers, and care teams to ensure safe, quality care
Requirements
Active Registered Nurse license in Michigan
Education: Associate degree in Nursing required; Bachelor’s degree preferred
CPR certification by American Heart Association (or willingness to obtain)
Minimum of one year of experience with frail, elderly, or long-term care population
At least one year of home care experience preferred; wound care experience preferred
Reliable transportation with a valid Michigan driver’s license and car insurance
Must be 18 years of age or older
Vaccination requirements or willingness to be fully vaccinated against influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B
Ability to work in community and home settings with potential interruptions and need for flexible scheduling
